About Home Based Services
Home Based Services is a Medicaid waiver program funded through the state of Illinois Department of Human Services. Individuals enrolled in this program will receive funding in the form of a monthly allotment, to be used to purchase services that will aide in achieving personal goals.
Dynamix offers Self-Direction Assistance (SDA) as an optional service for purchase through the waiver program. The SDA can assist in helping manage the monthly budget, enrolling in other programs that are authorized to use the monthly allotment, assisting in enrolling Personal Support Workers, benefits management, and obtaining information from various agencies state and local just to name a few.
The overall mission of the Home Based Services Program is to allow individuals to maintain independence while living at home.
